Chief Complaint: The patient complains of dysuria and some abdominal pain for the past 3 days History of Present Illness: N. T is a five year old uncircumcised male who was in his usual state of good health, now he presents with a complaint of dysuria and some abdominal pain for the past 3 days. The mother also reports high fever in the last 24 hours. It is crucial for NP to collect important subjective information following the seven attributes of symptoms to help guide the interview systematically (Swartz, 2014). This will aid to rule out acute condition requiring aggressive treatment (Dains et al., 2016). Since the patient is only five year old the mother would be the main historian, however, whenever is appropriate the child should also be included in the interview (Swartz, 2014).
